After the end of each discipline, the responsible teacher must ask the students who took the course to complete the student evaluation form, found here.

The form must be completed collectively by the class, by mutual agreement and must be delivered in a single copy, with everyone’s signature.
One form must be submitted per teacher. In other words, if the discipline had a responsible teacher, a form is delivered. If the subject had two responsible teachers, two forms, etc. are handed in.
Only after the students deliver the forms to the integrated secretariat, it will be able to enter the notes in the academic record.
